Position Summary
To provide comprehensive primary health care for unattached children and families living in the community, with an emphasis on service delivery, health promotion, and prevention based on their unique health care needs.
Key Areas of Responsibility
* Provide comprehensive, family-centred primary health care services within scope of practice.
* Provide assessment, intervention, monitoring, and follow-up for acute and chronic illnesses.
* Conduct client interviews and examinations; develop and implement client-centred treatment plans while integrating evidence-based health promotion practices.
* Provide seasonal, routine, and catch-up immunizations based on Ministry of Health publicly funded immunization schedules.
* Obtain, document, and interpret client information and maintain client records according to required medical record standards, ensuring privacy and confidentiality.
* Provide and receive consults on client care with other members of the interdisciplinary team.
* Adhere to College of Nurses standards of practice.
* Actively participate within an inter-professional model of care.
* Participate in peer and chart audit reviews as required.
* Participate in health promotion and outreach initiatives.
* Participate in case conferences.
* Work collaboratively in a team setting delivering services to marginalized individuals living with chronic illness, mental health challenges, and addiction.
* Participate in professional development opportunities and act as a preceptor for nurse practitioner and medical students when required.
* Participate in clinical team audits, quality improvement initiatives, and best practice activities within the organization and the broader community health team.
* Conduct home visits as required.
* Maintain competencies through ongoing professional development.
* Provide community education sessions as schedule permits.
* Participate in the on-call schedule as part of funder requirements.
Scope and Accountability
The Nurse Practitioner will function within their full scope of practice in accordance with College of Nurses of Ontario standards and is accountable to the Manager of Primary Care Services.
Educational and/or Professional Qualifications
* Master’s-prepared Nurse Practitioner with Primary Health Care certificate.
* Registration in good standing with the College of Nurses of Ontario in the Extended Class.
* Membership with the Nurse Practitioners’ Association of Ontario with additional professional liability coverage.
* Experience in program development preferred.
* Experience working as a Nurse Practitioner in a community or primary care setting strongly preferred.
* Oral and written fluency in additional languages relevant to the community served is considered an asset.
Level of Experience
* 3–5 years of practice as a Nurse Practitioner utilizing advanced nursing assessment skills in an inter- professional model of care (e.g., Community Health Centre, Family Health Team, or NP-led clinic).
* Current HCP CPR certification.
Skills and Attributes
* Ability to work independently with a high degree of self-direction and strong organizational skills.
* Excellent interpersonal, conflict resolution, and communication skills (written and oral).
* Proven ability to work effectively as a member of an inter-professional team.
* Familiarity with or willingness to work within an electronic health record documentation environment.
* Sensitivity and awareness of culturally, racially, economically, and socially diverse communities.
* Strong research, planning, and analytical skills.
Significant Working Conditions
* Extended hours may be required, including evenings and weekends, to ensure accessibility of services.
* Participation in an on-call rotation with physician support.
* Flexible working locations, including community-based settings.
* Valid driver’s license and access to a vehicle required.
Remuneration
$52.83 – $74.83 per hour
