Reporting to the Manager, Integrated Access, the Transition Services and Social Work Supervisor acts as a site-based leader responsible for the day-to-day operations of Transition Services and Social Work, including direct supervision of staff. This role collaborates with physicians, nurse practitioners, nursing staff, government representatives, community partners, hospital departments, and interdisciplinary team members to support coordinated care, patient flow, and timely transitions across the continuum of care. The position provides leadership in discharge planning, supports the resolution of complex patient situations, and fosters strong relationships with patients, families, and key stakeholders. The incumbent leverages a comprehensive understanding of healthcare, community, and government systems to facilitate access, transitions, and discharge planning initiatives. The Supervisor promotes interdisciplinary collaboration, coaches and develops staff, and supports continuous improvement in patient outcomes, access, and flow while modeling the Covenant Health Mission, Vision, and Values.

Required Qualifications:
Baccalaureate degree in a clinical discipline required. Current registration from the applicable professional regulatory body required. Current Heart and Stroke Foundation B.L.S. Certification (Level C) Health Care Provider required.
Additional Required Qualifications:
Minimum five (5) years recent acute and/or continuing care clinical experience required. Minimum of two (2) years of supervisory/management experience preferred. Exhibits strong clinical knowledge and skill in adult medicine, addictions and mental health, surgery, critical care, and geriatrics (Continuing Care, Supportive Living, Long Term Care) or relevant specialty. Knowledge of available community supports and programs.Combination of education and experience may be considered.
Preferred Qualifications:
