About the Role
Calgary Seniors is looking for an Outreach Worker to join our In Place Program.
The In Place Program supports older adults as they transition home from medical care. Through in-home visits, personalized assessments, short-term case management, and coordinated referrals, this role helps seniors stabilize after discharge, regain confidence, and remain safely connected to supports in the community.
The Outreach Worker bridges the gap between acute care and community-based supports, working closely with medical professionals, community partners, Calgary Seniors programs, and older adults themselves. This role is a strong fit for someone who is compassionate, organized, grounded under pressure, and skilled at helping people navigate complex systems during vulnerable moments.
What You’ll Do
In this role, you will:
- Receive referrals from the medical system and initiate contact with clients promptly.
- Conduct in-home assessments to understand immediate needs, risks, strengths, and goals.
- Create short-term transition plans that support safety, independence, confidence, and well-being.
- Provide hands-on support after community reentry, including help with transportation, groceries, home supports, referrals, applications, and appointments.
- Support older adults experiencing aging-related challenges such as grief, mobility changes, health concerns, isolation, or uncertainty after discharge.
- Coordinate services across healthcare, housing, transportation, food security, volunteer supports, and other community systems.
- Make warm referrals to Calgary Seniors programs and external long-term services.
- Advocate for client needs while supporting independence, accountability, and follow-through.
- Monitor client progress and adjust short-term plans as needed.
- Support smooth program exits, transitions, or handoffs when longer-term support is needed.
- Maintain accurate, timely documentation in internal systems.
- Track outcomes, client feedback, service gaps, and impact stories to support evaluation and reporting.
- Participate in Social Work team meetings, debriefs, training, agency events, and cross-department collaboration.
- Contribute frontline insights to program improvement, community collaboration, and strategic initiatives.
What You Bring
We are looking for someone with:
- A degree or diploma in Social Work or a related human services field.
- Active registration with the Alberta College of Social Workers, or eligibility to register, considered an asset.
- 3+ years of experience in case management, social work, community support, or human services.
- Strong understanding of senior-specific needs, mental health, aging-related transitions, and systems navigation.
- Experience supporting short-term case management, goal-based service navigation, or transition planning.
- Knowledge of the social determinants of health and their impact on older adults.
- Familiarity with social prescribing, health promotion, or community development frameworks considered an asset.
- Strong relationship-building, critical thinking, and problem-solving skills.
- Strong verbal and written communication, documentation, and planning skills.
- A trauma-informed, strengths-based, person-centered approach to working with clients.
- The ability to stay calm, grounded, and solution-focused in high-pressure or emotionally charged situations.
- Strong organizational and time-management skills, with the ability to manage competing priorities independently.
- Comfort working in homes, community spaces, and culturally diverse environments.
- Knowledge of seniors-serving community resources in Calgary.
- Comfort using Microsoft Office and digital documentation systems.
- A valid driver’s license, reliable vehicle, and appropriate insurance.
- A satisfactory Police Information Check and Vulnerable Sector Check.
- Legal entitlement to work in Canada.
Working Conditions
This is a full-time role based in Calgary, Alberta. The position is hybrid, with a strong in-office and community-based presence due to client visits, partner coordination, documentation, and program needs.
Local travel within Calgary and surrounding areas is required.
