About our Hospice:
Lisaard/Innisfree Hospice brings together an interdisciplinary team of trained professionals to deliver services that can make the dream of a peaceful, comfortable death, surrounded by loved ones, a reality and to fulfill the mission of hospice care by supporting residents and their families through the end-of-life journey.
The Registered Practical Nurse (RPN) is responsible for the nursing care of the residents in collaboration with other members of the palliative care team. The RPN functions independently within the scope of practice set by the College of Nurses of Ontario and appropriately seeks support from the Clinical Manager and physicians as required.
Hours of Work:
· Variety of shifts, including days, evenings, nights, weekends, and statutory holidays. Required to work at both hospice sites.
· Schedule published every four weeks. Opportunities are also available to pick up additional shifts.
Responsibilities include, but are not limited to:
· Develops, implements and evaluates an appropriate plan of care, in collaboration with the resident, family and health care team, which identifies objectives and anticipates outcomes as applicable.
· Documents timely and accurate reports of relevant observations, including resident and family teaching and outcomes, and prepares discharge summaries and transfer notes
· Processes physician orders.
· Acts as an advocate to protect and promote the resident’s rights to autonomy, respect, privacy, dignity and access to information.
· Protects residents and employees by adhering to infection control policies and procedures, medication administration and storage procedures, and controlled substance regulations.
· Acts as a role model to all staff by demonstrating positive behaviours and promoting teamwork.
· Coordinates work assignments of the PSWs by setting priorities, directing the plan of care to provide total care for the residents and housekeeping duties of the hospice as required.
· Willing to participate in the orientation of new staff and student preceptorship when required.
· Maintains a safe work environment by adhering to health and safety policies and procedures and ensures co-workers' practices are in compliance with the Ontario Health and Safety Act (OHSA).
· Is committed to life-long learning as demonstrated by attending educational programs to maintain and enhance clinical competency.
· Participates in staff meetings and in-services as often as possible.
· Understands the philosophy of hospice palliative care and has specialized education and/or experience in providing care to palliative clients.
· Participates in interdisciplinary committee work and projects as required.
· Accountable for maintaining a high level of confidentiality and privacy standards
· Participates in the Quality Assurance Program set out by the College of Nurses by annually performing a self–assessment and developing, implementing and evaluating a learning plan.
Requirements:
· Maintains current RPN registration with the College of Nurses
· Maintains CPR/AED certification annually
· Minimum of 1 year of palliative experience preferred; new graduates with student experience in a palliative setting will be considered
· Some community experience in palliative care an asset
· Completed the Fundamentals of Palliative Care Course and subsequent Enhanced Fundamentals course, or willingness to complete within 1 year from their date of hire
· Must possess basic computer literacy and a willingness to learn the electronic documentation used by the organization
· A Police Vulnerable Sector Check (PVSC) is required prior to the start date
