About the Role
Annapolis Valley First Nation (AVFN) is seeking a compassionate, dependable, and community-minded individual to join our Health Centre team as a Community Continuing Care Assistant (CCA) / Medical Transportation Driver.
This position combines direct client support with medical transportation services, helping community members maintain their independence, access healthcare services, and improve their overall well-being.
Schedule
30 hours per week, primarily daytime hours Monday to Friday with flexibility required to accommodate client and transportation needs.
Reports To
Community Health Registered Nurse (RN)
Key Responsibilities
- Provide personal support and assistance to clients in accordance with care plans.
- Assist with light housekeeping duties.
- Promote client dignity, independence, and well-being.
- Observe and report changes in client condition.
- Safely transport community members to and from medical appointments.
- Assist clients with mobility aids and wheelchairs.
- Complete vehicle inspections and maintain vehicle cleanliness.
- Maintain communication with Health Centre staff.
Qualifications
- Continuing Care Assistant (CCA) Certification or equivalent education and experience.
- Valid Nova Scotia Driver's License.
- Ability to obtain or currently possess a Class 4 Public Passenger License.
- Current CPR and First Aid Certification.
- Criminal Record Check and Vulnerable Sector Check.
- Strong communication and interpersonal skills.
Considered and Asset
- Experience working with seniors, individuals with disabilities, or community-based health services.
- Knowledge of Mi'kmaw culture and First Nations communities.
- Previous transportation experience.
