- Possess a compassionate and client-centered approach to caregiving
- Excellent communication and interpersonal skills
- Requires good judgement, problem-solving solving and assessment abilities
- The ability to work independently and cooperatively with home care coordinators
Key Responsibilities:
- Conduct comprehensive health assessments and monitor patient conditions
- Administer medications safely and accurately
- Provide wound care and monitor healing progress
- Manage chronic diseases and individualized care plans
- Deliver palliative and end-of-life care
- Assist patients with mobility and activities of daily living
- Provide respiratory care, including ventilator support and oxygen therapy
- Perform catheter and ostomy care
- Provide care for patients with G-Tubes and J-Tubes
- Manage tracheostomies and IV therapy
- Educate patients and their families on care routines, health management, and safety
- Experience working with pediatric patients is considered an asset
Qualifications:
- RN/RPN license valid in Ontario (Registered with CNO)
- Vulnerable Sector Check completed or willing to complete
- BLS Certification
