About the Opportunity
Oak's Loft Care Inc. is a Home and Community Support Service Provider serving London and Middlesex County. We provide compassionate, person-centered support to seniors, adults with disabilities, and individuals with complex care needs in their homes and throughout the community.
We are building a registry of experienced self-employed Registered Nurses (RNs) to provide on-call clinical consultation services on an as-needed basis. This is an Independent Contractor (B2B) opportunity with no guaranteed hours. Contractors are contacted only when clinical services are required.
Key Responsibilities
When assigned to a client, the Independent Practice RN will:
- Conduct post-hospital discharge assessments and monitor recovery.
- Assess wounds and provide advanced wound care using evidence-based practices.
- Perform advanced nursing foot care for eligible clients.
- Monitor for signs of infection and other health concerns.
- Provide clinical guidance to frontline care staff.
- Verify staff competency related to skin integrity, infection prevention, medication safety, and other delegated clinical activities.
- Maintain accurate clinical documentation in accordance with professional standards.
- Communicate assessment findings and recommendations to clients, families, physicians, and other healthcare professionals when appropriate.
Required Qualifications
Applicants must provide proof of:
- Active General Class registration with the College of Nurses of Ontario (CNO) in good standing.
- Advanced Nursing Foot Care certification and/or Advanced Wound Care certification (NSWOC or equivalent).
- Current Professional Liability Protection (minimum $2 million per claim).
- Independent WSIB clearance or applicable exemption.
- Valid Vulnerable Sector Check issued within the past 12 months.
- Current CPR/BLS certification.
- Valid Ontario driver's licence and reliable vehicle.
- Ability to travel throughout London and Middlesex County.
Preferred Experience
- Community nursing experience.
- Hospital discharge follow-up.
- Chronic disease management.
- Wound care.
- Foot care.
- Experience working independently in clients' homes.
