Position Summary:
The PACT Coordinator will coordinate clinical leadership and guidance to a multidisciplinary outreach team while providing patient care and provide daily clinical operations and outreach support.
Responsibilities:
- Provide operational and clinical leadership to the multidisciplinary PACT team, working closing with the Program Manager.
- Problem solving clinical support, emotional support and guidance for the entire multidisciplinary team including Program clerical support, Social Workers, Nursing Staff, Allied staff, Peer support staff.
- Liase with Team Psychiatrist and Manager on operational problems that may arise.
- Crisis intervention and support for staff and patients.
- Overseeing daily scheduling and problem solving with and for team.
- Represent and be the first point of contact for the program.
- Liason with inpatient unit to provide appropriate discharge plan.
- Liason with all other JBH Community Mental Health Programs, Community Partners Coast, Summit House, Halton Healthcare, Support and Housing Halton.
- To establish and maintain effective support systems for our clients.
- Liason with Act Ontario to ensure we are upholding best practices and standards as indicated.
- Intake and Case Management. Overseeing Ocan and all data submissions as required.
- Being the staff responsible for referals, intake and waiting list.
- Provide individual case management for small number of clients.
- Strong knowledge and understanding of ACT/PACT model.
- Monitor program performance, quality indicators, and fidelity to the model.
- Facilitate team meetings, case conferences, and care planning discussions.
- Participate in program planning, quality improvement initiatives, and accreditation activities.
- Support recruitment, onboarding, and professional development of team members in liaison with program manager.
Qualifications:
- Regulated health professional (e.g., Registered Nurse, Social Worker, Occupational Therapist, or other applicable regulated professional) with current registration in good standing with the appropriate regulatory college Must have a background in a discipline appropriate to the program. ie Nursing, Social Work, Occupational Therapy.
- Bachelor's degree in a relevant health discipline; Master's degree is considered an asset.
- Knowledge of Case Management, Psychopharmacology, crisis intervention and excellent communication skills are essential.
- Management skills as well as Supervision skills are an asset with previous leadership or supervisory experience is preferred.
