Job Summary

Responsibilities
- Conduct thorough patient assessments, including physical examinations and medical histories, with a focus on pediatrics, senior care, and acute care cases
- Develop and implement individualized treatment plans that incorporate pain management strategies and medication administration
- Collaborate with multidisciplinary teams to coordinate case management efforts and ensure seamless patient care transitions
- Manage patient documentation accurately using electronic health record systems such as Epic or Cerner
- Perform procedures such as phlebotomy and laboratory specimen collection with precision and adherence to safety protocols
- Provide education to patients and their families about health conditions, medication use, and preventive care measures
- Maintain current knowledge of medical terminology, assisted living protocols, dementia care practices, and home care procedures to deliver expert-level support
Requirements
- Valid Nurse Practitioner license with current certification to practice in your state
- Proven experience working with children, seniors, or in hospital settings with a strong emphasis on patient-centered care
- Experience in nursing home environments or assisted living facilities is highly desirable
- Background in acute care settings including emergency departments or hospital units
- Familiarity with medical terminology, medication administration protocols, and electronic health record platforms like Epic or Cerner
- Hands-on experience in pain management, case management, dementia care, laboratory procedures, and home health services
- Excellent communication skills for effective patient education and interdisciplinary collaboration
- Ability to handle multiple priorities efficiently while maintaining attention to detail
