POSITION SUMMARY
The Registered Practical Nurse (RPN) provides safe, compassionate, and evidence-based nursing care to residents and patients within a Acute Medical and Transitional Care setting. Working collaboratively within an interdisciplinary healthcare team, the RPN is responsible for assessing, planning, implementing, and evaluating nursing care for individuals with complex medical and personal care needs.
The RPN ensures care is delivered in accordance with professional nursing standards, organizational policies, and applicable legislation while promoting resident dignity, safety, independence, and quality of life.
The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible to provide excellent care to a diverse acute medical patient population in a fast-paced setting.
DUTIES AND RESPONSIBILITIES
- Provide direct nursing care including medication administration, treatments, wound care, and monitoring of residents’/patients’ health status.
- Conduct ongoing assessments and report changes in condition to the Registered Nurse (RN), physician, or appropriate healthcare provider.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary healthcare team.
- Support residents/patients with activities of daily living (ADLs) while promoting independence and dignity.
- Monitor and document clinical observations, interventions, and outcomes accurately and in a timely manner.
- Respond to medical needs and emergencies, initiating appropriate interventions within the RPN scope of practice.
- Collaborate with physicians, nurse practitioners, therapists, personal support workers, and other healthcare professionals to ensure coordinated care.
- Provide education and support to residents/patients and their families regarding care plans, treatments, and health management.
- Ensure infection prevention and control practices are followed according to organizational and public health guidelines.
- Participate in discharge planning and transitional care processes to support safe transitions between care settings.
- Supervise and provide guidance to Personal Support Workers (PSWs) and other care staff as appropriate.
- Maintain accurate and confidential health records in compliance with documentation standards and privacy legislation.
- Participate in quality improvement initiatives, staff meetings, and continuing education to support best practices in patient care.
- Takes a leadership role in the coordination of patient care in assigned area
- Other duties as assigned by Manager/delegate.
HEALTH & SAFETY

- Does not operate or use faulty equipment;
- Wears personal protective equipment (or clothing) as required by task, S.D.S. or residence policy;
- Promptly reports personal injury to supervisor and seeks first aid as needed;
- Participates in fire safety training and drills, and is familiar with the residence’s fire and emergency plans;
Required Education, Skills and Qualifications
- Current registration in good standing with the College of Nurses of Ontario.
- Diploma in Practical Nursing from an accredited educational institution.
- Current Basic Life Support (BLS) or CPR certification.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
- Proficiency with electronic health records and clinical documentation systems. Point Click Care, Mede Care, Point of Care, etc.
- Demonstrated commitment to resident-centered care and professional practice.
Working Conditions
- Ability to work shifts including days, evenings, nights, weekends, and holidays.
- Physical ability to assist with resident mobility, lifting, and repositioning in accordance with safe work practices.
- Exposure to a fast-paced healthcare environment requiring adaptability and strong organizational skills.
Job Types: Full-time, Part-time
Pay: From $28.41 per hour
