Job Summary
We are seeking a compassionate and dedicated Personal Support Worker (PSW) to provide exceptional care and assistance to individuals in various settings, including home, community, assisted living, and long-term care facilities. Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or resident’s personal assistive devices
- Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations
- Provide medication reminders and assist with medication administration as per care plans under supervision of nursing staff
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to ensure proper nutrition
- Support patients with dementia or Alzheimer’s by implementing specialized memory care techniques and ensuring a safe environment
- Maintain cleanliness of the client’s living space through light housekeeping, including cleaning routines aligned with infection control standards
- Document care activities thoroughly in compliance with healthcare regulations and update care plans regularly
- Assist with transportation to medical appointments or community activities, promoting social engagement and independence
- Engage in patient observation to detect early signs of health deterioration or behavioral changes for timely intervention
- Support hospice or palliative care clients by providing compassionate comfort measures and emotional support
Experience
- Previous experience working as a Personal Support Worker or in a similar caregiving role within home care, assisted living, nursing homes, or group homes is highly preferred
- Hands-on experience with patient monitoring, vital signs measurement, medication administration, and implementing care plans
- Group home experience or working with individuals with developmental disabilities or autism is advantageous
- Knowledge of dementia care, Alzheimer’s care, memory care techniques, and hospice support enhances your ability to deliver specialized services
- Familiarity with HIPAA regulations ensures confidentiality and privacy of patient information
- Experience using mobility aids such as Hoyer lifts or resident’s personal assistive devices is beneficial
- Strong communication skills for collaborating effectively with healthcare teams, patients, families, and social workers
