About the Client:
We're seeking compassionate 24-hour care for a woman in her senior apartment in Guelph who was recently diagnosed with stage 4 lung, spine, bone, and brain cancer. She lives independently in a one-bedroom apartment where meals are provided by the building. She has no immediate family nearby—no children or spouse—so consistent, reliable care is essential for family peace of mind during this palliative stage. She has a palliative care team that visits, including nurses for medical management, but needs dedicated PSW support around the clock. She uses a walker or wheelchair for mobility and is on high doses of morphine, which affects her memory and ability to manage daily tasks independently.
Responsibilities:
- Provide 24-hour supervision and safety monitoring
- Assist with personal care including bathing, dressing, and toileting
- Support mobility and transfers using walker or wheelchair
- Manage medication reminders and ensure proper administration (she sometimes forgets or double-doses)
- Provide meal reminders and light assistance with reheating food
- Help with light housekeeping, cleaning, and laundry
- Offer cognitive support including memory prompts, redirection, and companionship
- Coordinate with palliative care team as needed
- Maintain clear communication with family members who are managing care remotely
What We're Looking For:
- Experienced PSW with palliative or end-of-life care experience
- Comfortable working with clients who have memory changes and cognitive decline
- Reliable, consistent, and takes initiative when needed
- Skilled in personal care, mobility assistance, and medication management
- Calm, patient, and brings positive energy
- Strong communication skills with family and healthcare teams
- English speaking
- Post-hospitalization care experience preferred
- Able to work as part of a consistent care team on a set schedule
Schedule:
24-hour care coverage needed. We build consistent teams with the same caregivers on the same schedules each week—no revolving door of different faces. Specific shift times to be determined based on caregiver availability and family needs. Flexible start date, ideally within 1-2 weeks depending on discharge timeline from hospital.
