Position Summary
The Healthy Families Home Visitation Worker provides voluntary, relationship-based home visitation services to expectant parents and families with children from prenatal age through six years of age. The Home Visitation program is designed to strengthen parenting capacity, promote healthy child development, enhance parent-child relationships, and build family resilience through individualized, family-centred support.
Working from a strengths-based, trauma-informed, and culturally responsive approach, the Home Visitation Worker partners with families to identify their strengths, goals, and priorities while supporting healthy child development, positive parenting practices, and overall family well-being.
Services are delivered primarily in families' homes throughout Grande Prairie and surrounding rural communities, with flexibility to provide virtual or alternate methods of support when appropriate.
Key Responsibilities
Home Visitation & Family Support
- Build trusting, respectful, and collaborative relationships with expectant parents, caregivers, and families.
- Provide individualized home visitation services that promote healthy child development, positive parenting practices, and family well-being.
- Support caregivers in strengthening parenting knowledge, confidence, and skills through coaching, modelling, and guided parent-child interactions.
- Assist families in identifying strengths, setting goals, and developing practical strategies that support healthy family functioning.
- Support families in building protective factors, increasing resilience, and reducing the impacts of stress and adversity.
- Promote healthy attachment and nurturing parent-child relationships through evidence-informed practices.
- Encourage parent-led goal setting while respecting each family's unique values, culture, strengths, and priorities.
Child Development
- Monitor and support children's developmental progress through observation and developmental screening.
- Complete developmental and family screening tools in accordance with program requirements, including but not limited to:
- Ages & Stages Questionnaires (ASQ)
- Ages & Stages Social Emotional (ASQ:SE)
- Family Violence Screening
- Protective Factors or other approved assessment tools
- Share child development information with caregivers using evidence-informed resources.
- Support caregivers in understanding developmental milestones and age-appropriate expectations.
Family Service Planning
- Complete comprehensive family assessments and develop Individual Family Support Plans in partnership with families.
- Review and update Family Support Plans regularly to reflect family progress, changing goals, and emerging needs.
- Assist families in identifying barriers and developing realistic strategies to achieve their goals.
- Maintain regular contact with families based on program expectations and family needs.
Community Collaboration & Referrals
- Identify community resources and provide timely referrals to internal and external supports.
- Collaborate with community partners to ensure coordinated service delivery.
- Maintain current knowledge of available community programs and resources.
- Participate in case consultations and collaborative planning when appropriate.
Documentation & Administration
- Maintain accurate, timely, and confidential client documentation in accordance with organizational policies and funding requirements.
- Complete required case notes, assessments, service plans, data collection, and reporting within established timelines.
- Maintain professional records that accurately reflect services provided and family progress.
- Protect client confidentiality and adhere to all privacy legislation and organizational standards.
Team Participation
- Participate actively in team meetings, case consultations, supervision, and program planning.
- Contribute to continuous quality improvement initiatives.
- Support community awareness activities, outreach events, and program promotion as required.
- Participate on internal committees and external collaborative tables when requested.
- Perform other duties related to the position as assigned by the Healthy Families Team Lead or designate.
Qualifications
Education
One of the following is preferred:
- Diploma or Degree in Social Work, Human Services, Child and Youth Care, Early Childhood Education, Psychology, Family Studies, Nursing, or related fields. Equivalent education and relevant experience may be considered.
Experience
- Preferred experience includes:
- Working with young children and families.
- Home visitation or community-based family support.
- Child development and parenting support.
- Trauma-informed practice.
- Family assessment and service planning.
- Community resource navigation.
- Working with vulnerable or high-risk families.

Requirements
The successful candidate must maintain:
- A satisfactory Criminal Record Check with Vulnerable Sector Search.
- A satisfactory Intervention Record Check.
- Valid Standard First Aid Certification.
- A valid Class 5 Driver's License.
- Reliable vehicle for regular travel throughout Grande Prairie and surrounding rural communities.
- Minimum $2,000,000 automobile liability insurance with business use and valid registration.
- Ability to travel regularly and work independently in community settings.
Working Conditions
- Full-time position (35 hours per week).
- Work is performed primarily in family homes and community settings.
- Regular travel throughout urban, rural, and remote communities within the service area is required.
- Flexible scheduling may be necessary to meet the needs of families.
- Occasional lifting and transporting of program materials may be required.
Pay: $26.00-$27.00 per hour
