The Role:
Are you a compassionate and patient-focused Registered Practical Nurse with clinical coordination experience and excellent written and verbal communication skills?
KMH Cardiology Centers Inc. is growing, and we are recruiting for a Registered Practical Nurse (RPN)/Patient Care Coordinator for our busy specialist physician practices in Toronto and Scarborough.
The successful candidate will be part of a multi-disciplinary team whose goal is to support physicians, facilitate high-quality patient care outcomes, and coordinate clinical operations in a dynamic cardiology environment.
The Accountabilities:
The key accountabilities of this role are:
Care Coordination & Physician Support
- Coordinates in-clinic operations to support physicians.
- Manages physician orders and task workflow in a timely manner in accordance with established Key Performance Indicators (KPIs).
- Liaises with external facilities and service providers to coordinate patient referrals and ensure timely continuity of care.
- Serves as a liaison between members of the multidisciplinary team and external service providers to support high-quality patient care and optimal patient outcomes.
Patient Care & Communication
- Provides guidance, support, education, and advice to patients.
- Responds to patient enquiries regarding clinical care
- Informs and educates patients about tests and procedures as requested by the physician.
- Processes prescription renewal requests as directed by the physician, in collaboration with administrative staff
Documentation & Workflow Management
- Updates patient records and clinical documentation with clarity, accuracy, and completeness.
- Utilizes Tableau reports to manage daily workflow and prioritize activities.
- Meets established departmental and individual Key Performance Indicators (KPIs).
Collaboration & Teamwork
- Collaborates with cross-functional teams and other KMH staff to support exceptional patient care.
- Fosters a collegial, respectful, and collaborative clinic environment.
Required skills and experience:
· Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN)
· Minimum 1 year of nursing experience, preferably in a cardiology, specialist, or clinic setting
· Liability insurance coverage
· Experience in patient care coordination and clinical workflow management
· Proven ability to work proficiently in a busy, fast-paced specialist clinic environment
· Strong knowledge of medical terminology, with cardiology knowledge considered an asset
· Experience with electronic medical record (EMR) systems
· Proficiency and comfort with technology including MS Office
· Excellent verbal and written communication skills
· Good telephone etiquette and patient interaction skills
· Strong conflict resolution and problem-solving skills
· Self-directed, detail oriented with an ability to multitask, establish priorities and achieve objectives with minimal supervision
· Able to deal with sensitive information with a high degree of diplomacy and confidentiality
· BLS certification
· Team orientation with a collaborative approach to patient care
· The ability to provide coverage at KMH Toronto and KMH Scarborough

This is a full-time role for 5 days/37.5 hours per week scheduled as below:
Monday–Friday 8.00 am-4.00 pm
Saturdays are required on rotation with the team.
This position reports to the Manager, PCC and Clinical Support.
