About the Role
We are seeking a dedicated and patient‑focused Registered Family Nurse to join our virtual care team. The ideal candidate is experienced in primary care, comfortable providing assessments and follow‑up care through telehealth, and committed to delivering high‑quality, culturally sensitive patient support.
Key Responsibilities
- Conduct virtual nursing assessments, triage, and follow‑up care
- Provide patient education on chronic disease management, medications, and preventive health
- Support physicians and nurse practitioners with care coordination
- Review lab results, monitor patient progress, and escalate concerns appropriately
- Document all encounters accurately in the EMR
- Assist with virtual family practice services, including chronic disease follow‑ups, lifestyle counselling, and health maintenance
- Maintain confidentiality and adhere to professional nursing standards
Required Qualifications
- Active registration as an RN or RPN/LPN (depending on provincial regulations)
- Experience in family practice, primary care, or community health
- Strong clinical judgment and ability to work independently
- Excellent communication and virtual rapport‑building skills
- Reliable internet connection and private workspace
- Familiarity with telehealth platforms and EMR systems
Preferred Qualifications
- Experience in chronic disease management (diabetes, hypertension, obesity, mental health)
- Training in motivational interviewing or patient coaching
- Experience working with diverse and multicultural populations
- Previous virtual care experience
Pay: $25.00-$35.00 per hour
