Attention all RN/RPNs we have a position for you! We are looking for qualified RPN/RN nursing professionals to provide support in a Transitional Care Unit
Shift Details:
This is a temporary position. The anticipated duration is 12–18 months, after which the employee will transition to casual status.
Location: Kitchener
Shift timing: 7AM - 3PM (full time)
Pay Range: $34.85 - $38.36 per Hour

Responsibilities:
- Supervise activities of daily living and provide direction to the Personal Support Workers on the unit.
- Implement nursing care as outlined in the patient specific care plan.
- Communicate patient care needs and changes in patient status to the unit supervisor/Director of Nursing
- Provide direct patient care as outlined in the patient’s care plan including but not limited to administration of medication, completion of treatments, and patient teaching related to care needs.
- Complete patient admission
- Encourage active participation of patients in therapy programs and recreational activities.
- Liaise with physicians and other members of the interprofessional care team to ensure resident care needs are met
- Practice within the scope of practice set out by the College of Nurses of Ontario (CNO)
Requirements:
- Current registration with the CNO, candidate must be in good standing
- Valid work/study permit (if applicable)
- Current CPR/First Aid Certification
- Complete immunization record (MMR, TDAP, Varicella, Hep B)
- Clean Vulnerable Sector Screen (within 3 months of hire)
- Strong documentation and clinical skills
- High level of interpersonal skills
- Access to a vehicle is considered an asset
