Position: Community Intake and Resource Coordinator, 12-Month Maternity Leave Contract, Full-Time
Compensation: $50,000-$58,000
Since 1995, Cortellucci Hospice Vaughan has supported individuals and their families, as they journey through a life-limiting illness. Our Hospice Palliative Care Hub includes a 12-bed hospice residence that provides a final peaceful homelike space for individuals and their families who require end of life support. In addition, we offer a range of support programs for our community, including grief and bereavement support.
JOB PURPOSE AND GENERAL ACCOUNTABILITY
This role will appeal to a dynamic and experienced regulated healthcare professional who values the community and who is eager to support a growing organization and an energetic team. Reporting to the Manager of Community Services, the Community Intake and Resource Coordinator is responsible for evolving our programs by building new partnerships and strengthening existing community alliances to optimize who and how we support clients in the community. The Community Intake and Resource Coordinator applies their knowledge of formal (e.g., Health & Allied Health, Community, Social Service networks) and informal services (e.g., neighbourhoods, cultural groups, faith-based groups) to support individuals and families based on their needs and goals of care.
The Community Intake and Resource Coordinator will work closely with the direct service team to evolve the triage system for client intake and referrals, mapping out structured care pathways to and from long-term care homes, hospitals, retirement homes, older adult clubs, multi-faith centres, and other community support services. They will identify the needs of individuals living with a life-limiting illness, their caregivers, and those grieving the loss of a loved one. This role will work in partnership with the service team to identify and develop appropriate supports and participation in programs.
DUTIES AND RESPONSIBILITIES
Direct Service:
· Receive inquiries and referrals, connect with clients to complete an intake assessment to determine needs and eligibility, and establish a personalized plan of care with each client
· Monitor and evaluate client care plans at regular intervals to assess changing needs
· Connect clients with information and services to support their health and well-being and offer support to advocate for client needs
· Manage confidential client records through paper and electronic systems, including the use of InfoAnywhere, in compliance with regulations and professional standards including the Personal Health Information and Privacy Act (PHIPA)
· Attend daily palliative care huddles
· Manage the Wellness Program in collaboration with the Volunteer Coordinator, ensuring a smooth process for clients and volunteers
Outreach Services:
- Collaborate with other agencies and professionals for the benefit of the client(s)
- Identify gaps in service
- Advocate for system change
- Organize and deliver public education as required
- Identify collaborative partnerships and build strategic outreach relationships to support operations and ensure harmonized planning
Other Activities:
- Assist in the development and implementation of new programs and services to support increased client and family access of blended supports in one space
- Promote the services of Cortellucci Hospice Vaughan in a variety of settings
- Attend team meetings, staff meetings, and employee engagement initiatives
- Participate in organizational events and activities
- Assist with general office functions as needed
- Other duties as assigned
Position Requirements:
- Regulated healthcare professional, such as a Registered Nurse or Registered Social Worker; master’s preparation is an asset
- Palliative care education and/or CNA certification in hospice palliative care is preferred
- Experience in community hospice palliative care
- Ability to work autonomously and collaboratively with members of the interdisciplinary team
- Ability to comply with all federal, provincial, and municipal regulations and legislature
- Self-motivated, result-oriented, personable, diplomatic, and the ability to multitask
- Ability to work flexible hours and to commute between various locations in Vaughan
- Understanding of ethics as they relate to hospice palliative care
- Excellent communication, interpersonal, organizational, and time management skills
- Knowledge of community resources
- Experience collaborating with volunteers is desirable
- Ability to take initiative and to respond efficiently and professionally to requests for information
- Ability to maintain confidentiality and meet deadlines
- Experience working with diverse groups of people and volunteers
- Experience in program development and evaluation is an asset
- Access to a reliable vehicle and valid insurance is an asset
