Volunteer Coordinator / Community Client Coordinator
Job Summary
This dual-role position provides holistic, client-centered hospice support within the community while also leading the recruitment, training, engagement, and coordination of hospice volunteers. The successful candidate will work as part of an interdisciplinary team to support individuals and families facing life-limiting illness, while ensuring volunteers are effectively matched, supported, and engaged in meaningful service.
Responsibilities of Community Hospice Coordination
In this aspect of the role, the Coordinator supports clients and caregivers through comprehensive, holistic care planning and system navigation.
Responsibilities include:
- Assessing clients and caregivers to identify physical, clinical, psycho-social, and spiritual needs
- Developing, implementing, and monitoring individualized, holistic care plans
- Providing psycho-social support, education, and guidance to clients and families
- Supporting system navigation and connecting clients to community and healthcare resources
- Conducting regular check-ins with clients and caregivers to monitor changing needs
- Maintaining accurate and timely documentation, case notes, and client records
- Collaborating with volunteers and team members to support care delivery and safety in the home environment
- Supporting caregiver well-being through education, resources, and connection to supports
- Participating in program reporting, including annual client reviews and audits for accreditation purposes
- Engaging in interdisciplinary teamwork to ensure coordinated, high-quality hospice care
Responsibilities of Volunteer Coordination
In this aspect of the role, the Coordinator is responsible for the full cycle of volunteer engagement and support.
Responsibilities include:
- Recruiting, interviewing, and onboarding new volunteers aligned with hospice values and mission
- Providing orientation, training, supervision, and ongoing support to volunteers
- Matching volunteers to appropriate roles based on client needs, interests, and availability
- Maintaining ongoing communication with volunteers through meetings, check-ins, and updates
- Facilitating volunteer support meetings, education sessions, and wellness initiatives
- Managing volunteer records, including annual reviews and documentation updates
- Supporting volunteer recognition, appreciation events, and awards
- Collaborating with staff and leadership on volunteer utilization across programs and events
- Gathering and responding to volunteer feedback to support continuous improvement
- Supporting reporting requirements, including annual volunteer audits for leadership and Board review
- Working within an established budget to support volunteer engagement and initiatives
Qualifications
- Experience in health, social services, community care, or related field
- Experience in hospice, palliative care, and/or volunteer coordination considered an asset
- Ability to work independently, including conducting home visits as part of routine service delivery
- Strong interpersonal, communication, and organizational skills
- Ability to manage competing priorities in a community-based setting
- Knowledge of client-centred and holistic care approaches
- Strong documentation and computer skills
