Employer: Proof of Care (In-Home Care Provider)
Schedule: Evening shifts Monday to Friday (6:00 PM – 8:00 PM) and weekend shifts Saturday and Sunday (8:00 AM – 10:00 AM, 1:00 PM – 3:00 PM, and 6:00 PM – 8:00 PM). Nurses may choose the shifts that best fit their schedule. Preference will be given to those who can provide consistent coverage.

About the Role
Proof of Care is currently seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-to-one care for a private client in North Vancouver.
This position focuses primarily on G-tube feeding and medication administration in a home setting. The successful candidate will be comfortable working independently, following established care plans, and maintaining close communication with the client, family, and Proof of Care team.

Key Responsibilities
Provide G-tube feedings according to the prescribed schedule and care plan.
Administer medications through the G-tube safely and accurately.
Prime feeding equipment, flush tubing before and after feeds and medications, and ensure proper operation of feeding equipment.
Monitor the client during and after feedings for any signs of intolerance, aspiration risk, discomfort, or complications.
Troubleshoot common G-tube concerns, including occlusions, feeding interruptions, equipment issues, and site concerns, while reporting significant findings as appropriate.
Perform routine assessments and monitor changes in the client's condition.
Maintain accurate documentation of feedings, medications, observations, and nursing interventions.
Communicate effectively with family members, healthcare providers, and the Proof of Care team.
Follow infection prevention and control practices and ensure a safe care environment.
Required Qualifications
Licensed Practical Nurse (LPN) or Registered Nurse (RN) degree/diploma from a recognized Canadian nursing program.
Current registration and good standing with the British Columbia College of Nurses and Midwives (BCCNM).
Previous experience with G-tube feeding and medication administration through feeding tubes.
Knowledge of G-tube care, including priming, flushing, monitoring, troubleshooting blockages, and identifying potential complications.
Current CPR and First Aid certification.
Valid Criminal Record Check, including Vulnerable Sector screening.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently in a private home setting.
Preferred Qualifications
Previous home care or community nursing experience.
Experience caring for seniors or medically complex clients in the community.
Comfort working collaboratively with family members and other healthcare professionals.
Flexible availability and willingness to provide ongoing consistent coverage.
