Transitional Housing Co-ordinator

Phoenix Rising is a multi-faceted organization that provides services to all women with or without children including those experiencing concerns with mental health, addictions and/or domestic violence. As an organization we operate within a feminist framework, in which equality for women is encouraged, the practice of women helping women is promoted, and individual differences are respected. Phoenix Rising consists of an apartment building, drop-in centre and our new Bridge units located at 235 Wellington St West that will provide services to the 8 transitional supportive housing units Monday to Friday, 8:30 to 4:30.
Direct Service
· Provide daily support and services focusing on life skills to all (8) program participants.
· Ensure all demographics and assessments are documented in Hifis.
· Responsible for planning programing, life skill workshops and facilitating the programming.
· Make long lasting partnerships with community partners
· Work closely and in tandem with Paulines Place.
· Ensure all program participants are following the Pathway to Home Agreement.
· Work alongside the Homelessness Prevention Team (HPT) by attending monthly meetings to ensure appropriate services and supports are in place for individuals residing within the Pathway to Home Program.
· At each HPT meeting provide weekly updates including programming (life skills, social/recreational, etc) and updated assessments for the eight (8) supported individuals within the Pathway to Home Program.
· Administer move in SPDAT, regular interval SPDAT assessments (30, 60, 90 day) and ensure they are uploaded into Hifis.
· Coordinate safe entry and exit of individuals residing in and/or representatives from community agencies visiting the Pathway to Home Program located within the building located at 235 Wellington Street W.
· Restrict non-residents access to the facility.
· Provide daily meals in accordance with Canada’s food guide.
· Report any incidents and/or disturbances to the appropriate authority including police and housing services.
· Report any maintenance issues to Housing Services.
Qualifications
- Degree or diploma in Social Services or related field.
- Excellent understanding of feminist philosophy and a demonstrated ability to work within that framework.
- Knowledgeable of mental health, homelessness, addictions, domestic violence, intergenerational trauma and issues relating to women.
- Knowledge of community agencies, provincial, national, and international partners both for profit and not-for-profit agencies and organizations related to women for referral.
- Excellent interpersonal skills.
- 3-5 Years experience in social services.
- Valid G drivers license, access to a reliable vehicle.
- Both vulnerable sector check and criminal record check required.
- CPR and First Aid Certification- required.
Salary- $28 per hour/35 hours per week.
