Job Summary
we are hiring part-time Nurse Practitioners for our client, a leading primary healthcare provider. The ideal candidate will provide comprehensive, family-centred primary health care to unattached children and families in the Scarborough community, with a focus on accessible service delivery, health promotion, disease prevention, and care tailored to individual and family health needs.
Key Responsibilities
- Deliver comprehensive, family-centred primary health care services within the scope of practice.
- Assess, manage, monitor, and follow up with clients experiencing acute and chronic health conditions.
- Conduct client assessments, interviews, and physical examinations; develop and implement individualized treatment and care plans using evidence-based health promotion strategies.
- Administer seasonal, routine, and catch-up immunizations in accordance with the Ministry of Health’s publicly funded immunization schedule.
- Collect, document, and interpret client health information while maintaining accurate medical records in accordance with the healthcare facility's standards, privacy legislation, and confidentiality requirements.
- Collaborate with interdisciplinary team members by providing and receiving consultations regarding client care.
- Practice in accordance with the standards, guidelines, and professional requirements established by the College of Nurses.
- Contribute actively to an interprofessional model of care and participate in collaborative decision-making.
- Participate in peer reviews, chart audits, clinical audits, and quality improvement activities as required.
- Support the Health Centre’s health promotion, community outreach, and public education initiatives.
- Participate in multidisciplinary case conferences and care planning activities.
- Provide culturally responsive and client-centred services to marginalized populations, including individuals living with chronic illness, mental health concerns, and addictions.
- Participate in professional development activities and provide preceptorship and mentorship to Nurse Practitioner and medical students when required.
- Contribute to the implementation of evidence-based practices, quality improvement initiatives, and clinical best-practice activities within the organization and broader community health network.
- Conduct home visits when clinically appropriate and required.
- Maintain professional competencies through ongoing education, training, and continuing professional development.
- Deliver community education sessions and health-related information programs as scheduling permits.
- Participate in on-call rotation in accordance with organizational and funder requirements.
Qualifications
- Master’s-prepared Nurse Practitioner with a Primary Health Care certificate.
- Registered in good standing with the College of Nurses of Ontario (CNO) in the Extended Class.
- Member of the Nurse Practitioners’ Association of Ontario (NPAO) with additional professional liability coverage.
- Experience in program development is preferred.
- Previous experience working as a Nurse Practitioner in a community-based setting is strongly preferred.
- Willingness to join additional professional organizations to support and enhance professional practice, such as the Registered Nurses’ Association of Ontario (RNAO).
- Fluency in additional languages relevant to the community and services provided is considered an asset.
- Demonstrated ability to work independently with a high degree of initiative, self-direction, and strong organizational skills.
- Strong interpersonal and communication skills, including demonstrated ability in written and verbal communication and effective conflict resolution.

Pay: $130,000.00-$135,000.00 per year
