Registered Nurse or Registered Psychiatric Nurse Case Manager (CM) - Mental Health [Virtual]
At Cognito Health, we seek a dedicated Registered Nurse Case Manager to join our mental health team. As a Case Manager, you will play a crucial role in supporting our clients through their journey to mental wellness alongside a team of Nurse Practitioners, CBT Care Providers, and Psychiatrists. Your responsibilities will include patient intake assessments, lab/ECG review, and case management for our programs focused on anxiety, depression, insomnia, and ADHD.
Key Responsibilities:
- Intake Appointments: Conduct thorough patient intake assessments independently, gathering comprehensive histories and introducing patients to our Cognito program. Consult NPs / NP lead / team psychiatrist when necessary to confirm lab requirements / patient eligibility.
- Lab Review and Communication: Review laboratory and ECG results prior to intake appointments and communicate abnormal results as necessary.
- ADHD Scale Review: Assess ADHD / PHQ9 / GAD7 rating scales in collaboration with Nurse Practitioners as needed.
- Case Management: Provide comprehensive case management for patients discharged or escalated due to urgent concerns, collaborating with multidisciplinary teams for discharge planning and post-care support.
Qualifications and Skills:
- Valid RN license in British Columbia, Canada,required
- Valid RN license in Ontario, Canada, an asset
- Bachelor of Science in Nursing (BScN) preferred
- Experience in psychiatric or mental health nursing is an asset
- Strong interpersonal and communication skills
- Ability to work collaboratively within a multidisciplinary team
- Critical thinking and problem-solving abilities
- Compassionate and patient-centered approach to care

Employment Structure:
This position will initially be offered as a contractor role for a period of 3 months, with the potential to transition to an employee role based on performance, mutual agreement, and company needs. During the contractor period, you will have the flexibility of remote work with expected hours of 20-30 per month with the opportunity to increase based on business needs and client demand.

Pay Range:
$48.00 to $57.00 per hour, commensurate with experience.
