Duties & Responsibilities
- Provides client–centered primary care supports as outlined and appropriate within the RN scope of practice.
- Undertakes triage responsibilities and performs intakes to the primary care unit. • Active involvement in the overall coordination of patient care.
- Reports and records accurately and appropriately, using EMR charting.
- Administers immunizations.
- Effectively delivers chronic disease management.
- Demonstrates excellent skills and knowledge of well-baby growth and development with ability to apply this knowledge to nursing care.
- Demonstrates excellent skills and knowledge of women’s reproductive health care and screening, antenatal care, intrapartum care, immediate postpartum care, breastfeeding support.
- Facilitates discussion with clients to determine existing and potential health concerns with a focus on related lifestyle, psychological and environmental risk factors and assessments, to stabilize and improve client’s health status.
- Works within established care pathways, medical directives, guidelines and protocols.
- Consults with Traditional Healers when indicated and is willing to learn about Traditional approaches to health and wellbeing.
- Works independently and collaboratively with members of the interdisciplinary team and consults with RN(EC) and Physicians.
- Completes follow-up required by client and/or refers when appropriate.
- Assists with lab tracking and follow up.
- Maintains data required by Anishnawbe Health Toronto for reports and proposals.
- Performs other related duties as required.
Qualifications & Experience:
- Current Certificate of Registration with the College of Nurses in Ontario (CNO) as a Registered Nurse in good standing required.
- Diploma or Baccalaureate degree in nursing required.
- At least 2-5 years of experience providing patient care to patients of all ages in a comprehensive community health center setting.
- Previous experience with an electronic medical record (EMR).
- Knowledge of clinical best practices for the care and treatment of clients with complex medical and co-morbid conditions. • Knowledge and experience with mental health care and substance misuse is an asset but willingness to learn is critical.
- Excellent written and verbal communication skills and relationship building skills.
- Commitment to working cooperatively in a community-based setting as part of an interdisciplinary team; ability to collaborate within and across internal and external systems playing a vital role in the delivery of services.
- Demonstrated competence with EMR and strong documentation skills.
- Demonstrated satisfactory work performance and attendance history.
