Key Responsibilities

Use best practice and innovative approaches to provide client-centered services including assessment, treatment and psychosocial rehabilitation.

Conduct comprehensive client assessments in collaboration with clients.
- Maintain workload measurement and data collection as required.

Regularly update client service plans (including crisis plans).

Provide direct assistance and support to clients regarding substance use, housing, employment, relationships & leisure activities, activities of daily living, and transportation.

Work collaboratively, cooperatively and effectively within the team and with community partner agencies including hospital inpatient units and emergencies.

Monitor and supervise clients in the community as per their Ontario Review Board disposition orders.

Participate in formulation of risk assessment and management plans with clients.

Provide crisis assessment and intervention to clients.

Participate in team meetings to review clients and organize daily assignments.

Advocate with community partners for services and opportunities for clients.

Keep current with hospital policies, procedures, and applicable legislation (Mental Health Act etc.).
Qualifications
-
B.A. in Social or Health Sciences is required.

Experience with Recovery principles and practice.

Knowledge of applicable legislated acts (i.e. MHA, The Health Care Consent Act, Criminal Code of Canada).

Excellent communication skills, organizational ability, critical thinking and goal setting skills.

Excellent observation, assessment and crisis intervention skills.
-
Group leadership skills and experience.

Excellent knowledge of community resources (housing, finance etc.).

Valid driver’s license and access to reliable transportation.

Maintain workload measurement and data collection as required and input into the workload measurement system on a regular basis.
Language Requirements

English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is preferred.

Salary Range:
45.06 to 52.33 per hour
