Mind Connections Inc. is an Ontario based multi-disciplinary rehabilitation company, providing services to severely injured individuals including ones with traumatic brain injuries, spinal cord injuries, severe orthopedic injuries and mental health issues.
Our team comprises of neuro-surgeons, orthopedic surgeons, neurologists, psychiatrists, physiatrists, psychologists, occupational therapists, social workers, speech language pathologists, dietitians, rehabilitation support workers and personal support workers working collaboratively with the clients. Our occupational therapists complete discharge planning assessments for clients in hospital or rehabilitation settings, attendant care assessments, functional cognitive assessments, catastrophic, GOS-E assessments and future care costs reports. We do accessibility assessments to address home modifications for wheelchair accessible housing and return to work assessments. We provide occupational therapy treatment including cognitive training, community integration and functional rehabilitation to the clients in their environment to maximize the functional performance of our clients.

We are currently seeking a motivated and enthusiastic Occupational Therapist to join our team.
Qualifications:
- Entry level Masters degree or higher in Occupational Therapy
- Strong writing and communication skills
- Critical thinking and decision making skills
- Effective time management and organizational skills
- Experience working under the Statutory Accident Benefits Schedule (SABS) will be considered an asset.
Responsibilities will include:
- Conducting In-home assessments, discharge planning assessments, functional cognitive assessments and catastrophic assessments in a community setting based on presenting impairments, disability, or handicap, using standardized assessment tools
- Formulating treatment plans using evidence-based information, relevant best practice guidelines and clinical expertise
- Actively engaging clients, families, and caregivers in treatment programs to address client centered goals
- Linking clients/caregivers with appropriate community partners/services/agencies
- Evaluating and documenting the effectiveness of the intervention, client’s response, and progress towards goal attainment
- Completing clinical records according to college standards
- Communicating effectively with clients, families, referral sources including team members and office staff.
