The Opportunity:
We are recruiting for a Service Resolution/Service Solutions Facilitator (Children’s Mental Health & Developmental Services and Adult Developmental Services)
- This is a regular full-time position (35 hours per week Monday to Friday 9am to 5pm with some flexibility to meet service needs) based at 737 Woolwich St, Guelph with travel as required.
Reporting to the Children's Services Manager, the Service Resolution/Service Solutions Facilitator is responsible for leading a collaborative process with multi sector partners to resolve complex individual/family situations with extraordinary needs across Wellington and Dufferin counties. This position serves children in the mental health and developmental services sectors, and adults in the developmental services system. The Service Resolution/Service Solutions Facilitator works with members of the Wellington Dufferin Service Resolution/Service Solutions Steering Committee to resolve individual/family complex situations, manage specialized funding, as well as identifying themes and trends in the service system. The Service Resolution/Service Solutions Facilitator participates in various planning initiatives to enhance the system locally, as well as within Central West Region. The Service Resolution/Service Solutions Facilitator demonstrates strong skills in facilitation, collaborative care planning, financial management, relationship building, negotiating, understanding of complex clinical situations, communications and systemic thinking and practice.
The incumbent requires:
- Masters level degree in psychology, social work or relevant health or social sciences field
- A minimum of 3 years related experience
- Demonstrated success in the application of community-based services, and an understanding of the challenges and importance of collaborative service ventures
- Experience facilitating case conferences
- Knowledge of children's and adult developmental service systems, including Developmental Services Ontario (DSO)
- Comprehensive knowledge of regional and catchment area service systems
- Demonstrated understanding and sensitivity to social and cultural diversity
- Highly developed organizational, communication and problem solving skills
- Understanding of budget formulation and basic financial management an asset
- Proficiency with MS Office, Outlook and various database software
- Valid driver’s license and reliable transportation

CLASSIFICATION AND RATE OF PAY: Level 8 (Non-Union) - $46.37 TO $56.33 per hour
