What You'll Do
As a Health Care Aide with Ascent Home Care, you'll provide personalized support that helps clients remain safe, comfortable, and independent in their own homes.
Depending on the client's care plan, your responsibilities may include:

Personal care, including bathing, dressing, grooming, toileting, and mobility assistance

Medication reminders within your scope of practice

Observing and reporting changes in a client's health, behaviour, or condition

Meal preparation and assistance with eating when required

Light housekeeping related to the client's care and immediate environment
-
Companionship and meaningful social interaction
-
Supporting established routines that promote comfort, dignity, and independence

Following individualized care plans and documenting care accurately
-
Communicating clearly with clients, families, and the Ascent nursing team
-
Reporting concerns or changes promptly so our clinical team can respond appropriately
Qualifications

Health Care Aide certificate or recognized equivalent
-
Current First Aid and CPR certification
-
Ability to provide a satisfactory criminal record check, including Vulnerable Sector Check
-
Strong verbal and written English communication skills
-
Professional, dependable, and comfortable working independently in clients' homes
-
Ability to follow care plans, document accurately, and communicate changes in client condition
-
Valid driver's licence and access to a reliable vehicle are considered assets
-
Additional languages are considered an asset
