Job Summary
Join our compassionate team as a Caregiver / Health Care Aide and make a meaningful difference in the lives of individuals who require support in their own homes. In this role, you will provide personalized, respectful care to seniors, individuals with disabilities, and clients requiring home or palliative support.
Your compassionate and dependable approach will help promote independence, dignity, comfort, and quality of life for each client you support. You will work closely with clients, families, caregivers, and our Director of Care/RN to ensure care is provided according to each client's individualized care plan.
Responsibilities
- Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility, while maintaining dignity and privacy.
- Provide assistance with mobility, transfers, and positioning according to the client's care plan and your training.
- Provide medication reminders and support clients in following their established medication routines, where applicable and within the scope of your role.
- Prepare meals according to individual dietary needs, preferences, and care plans.
- Perform light housekeeping tasks, including laundry, tidying, and maintaining a clean and safe home environment.
- Provide companionship and emotional support to help reduce isolation and promote social well-being.
- Assist clients with daily routines and activities that encourage independence.
- Observe and report changes in the client's condition, behaviour, or needs to the appropriate member of the care team.
- Maintain accurate and timely documentation of care provided.
- Communicate effectively with clients, families, caregivers, and the multidisciplinary care team.
- Follow individualized care plans, company policies, safety procedures, and infection-prevention practices.
- Maintain client confidentiality, dignity, and professional boundaries at all times.
Qualifications & Experience
- Health Care Aide (HCA) certification is required; relevant caregiving experience may also be considered.
- Previous experience in home care, assisted living, long-term care, or a similar caregiving environment is preferred.
- Experience supporting seniors, individuals with disabilities, dementia, or palliative/end-of-life clients is an asset.
- Current First Aid & CPR certification.
- Vulnerable Sector Search (VSS) required or willingness to obtain one.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with clients, families, caregivers, and the multidisciplinary care team.
- Reliable transportation and the ability to travel to clients' homes.
- Dependable, punctual, compassionate, and professional.
- Ability to work independently while following established care plans and instructions.
- Ability to perform physical tasks associated with caregiving, including assisting with transfers and mobility, in accordance with proper training and safe-work practices.
