POSITION SUMMARY
The Community Outreach Worker provides relationship-based, practical support to LNIB Members across the lifespan to promote independence, stability, and overall wellbeing. This role focuses on systems navigation, life skills development, and community connection, working alongside other Human Services staff to ensure members receive coordinated, dignified, and culturally safe support.
The Community Outreach Worker supports individuals and families in navigating services, building capacity, and strengthening their connection to community resources. This role is intentionally collaborative and works closely with the Youth Outreach Worker where services and populations overlap.
KEY RESPONSIBILITIES
- SYSTEMS NAVIGATION & INDIVIDUAL SUPPORT
o Work one-on-one with community members to assess needs, identify priorities, and develop practical support plans.
o Support access to internal and external services including housing, employment and training, addictions services, mental health support, medical services, and cultural supports.
o Provide warm handoffs, follow-up, and coordination to reduce service gaps or duplication.
o Advocate alongside members while supporting self-advocacy and informed decision-making.
- LIFE SKILLS MENTORSHIP & COACHING
o Provide practical mentorship and coaching in areas such as budgeting, household organization, parenting skills, personal care, housing stability, and goal setting.
o Support individuals and families in building confidence, resilience, and self-sufficiency.
o Take a strengths-based, trauma-informed approach that respects individual readiness and lived experience.
- COMMUNITY ENGAGEMENT & GROUP SUPPORT
o Encourage and support participation in community programs, activities, and cultural events.
o Reduce isolation by fostering connection and sustained engagement beyond crisis response.
o Facilitate or co-facilitate life skills workshops and community education sessions.
- COLLABORATION & INTEGRATED SERVICE DELIVERY
o Work collaboratively with the Youth Outreach Worker and other Human Services staff to coordinate support.
o Participate in joint planning, case coordination, and shared programming where appropriate.
o Maintain clear documentation and communication to support integrated service delivery.
- CULTURAL FOUNDATIONS & PRACTICE
o Demonstrate respect for Nlaka’pamux values, traditions, and cultural teachings.
o Embed cultural safety, relational accountability, and dignity-based practice into daily work.
o Engage in ongoing learning related to culture, trauma-informed practice, and community wellness.
KNOWLEDGE, SKILLS & ABILITIES
- Strong interpersonal and relationship-building skills.
- Ability to support individuals with complex and intersecting barriers.
- Knowledge of community-based services, systems navigation, and advocacy.
- Ability to work independently while collaborating within a multidisciplinary team.
- Strong communication, organization, and documentation skills.
- Commitment to culturally safe, trauma-informed, and strengths-based practice.
QUALIFICATIONS
- Diploma or degree in Human Services, Social Work, Community Support, or a related field.
- Relevant experience working with First Nations communities and individuals facing social, economic, or systemic barriers.
- An equivalent combination of education and lived or professional experience may be considered
WORKING CONDITIONS
- Community-based role with flexible scheduling.
- May include evening or weekend activities aligned with youth programming.
- Exposure to emotionally demanding situations.
- Must maintain a current and clear Criminal Record Check.
