TransCare Community Support Services is a charitable not-for-profit corporation dedicated to providing home and community support services for people who need assistance due to illness, convalescence, disability, or functional limitations related to aging. TransCare® offers a continuum of services to facilitate independent living, comfortably and safely in the clients’ homes, and in the community.
Position Overview:
To provide referral and service coordination, crisis intervention, home visits, and case management to senior and adults with disabilities, in their efforts to maintain independence in the community. This job will require a strong person who is able to work in partnership with CCAC and Hospitals, as well as, Agency team members.
Primary Duties & Responsibilities:
- Advocacy and CSS navigation to support clients with one-on-one case management that will involves conducting telephone and face to face interventions in the community;
- Provide need-specific support to clients facing vulnerability or crisis, including neglect, abuse, mental health and crisis;
- Guide decision making and care planning through education and linkage to supports for clients and their families/ caregivers;
- Make referrals to other community resources such as recreational and social programs, support groups, and health and wellness programs;
- Conduct on-going assessment and reassessment of client needs and support progress, including fee subsidy eligibility for active clients;
- Participate in regular community outreach, engagement events and promotion of Agency programs;
- Provide resources for diverse clients looking for ethnic-specific programs and education;
- Maintain detailed documentation on AlayaCare client database and maintain updates of client charts in accordance and in compliance with agency’s confidentiality and privacy policies;
- Provide regular statistical, verbal and/or written reports to the Program Director or designate on issues related to the program;
- Provide active team support and back up for Home At Last Program, Home First and Supported Referral roles and supporting the Agency in their lead roles in these initiatives;
- Collaborate with agency staff for case conferencing for common clients;
- Maintain collaborative focus and inter-professional relationships with other health care professionals and key partners, including active participation on interdisciplinary teams both internally and externally;
- Regularly attend networking meetings and maintaining commitment in representation in new and on-going initiatives;
- Perform other duties and responsibilities as directed by Supervisor.
Job Specifications: (Minimum Knowledge/Education, Experience, Skills and Abilities normally required for competent performance in the job)
- Degree in Social Work or Diploma in Social Service Work;
- Familiarity with RAI-CHA assessments, IAR as well as Health Links initiatives;
- Minimum of two years’ experience in CIA coordination or case management functions;
- Knowledge and understanding of Gerontology -- the aging process, the specific needs of the elderly, disabled and caregivers;
- Maintain professionalism and objectivity in day to day roles;
- Strong familiarity with community support services as well as key entities and continuum of care, such as hospitals, rehab, long term care;
- Sensitive to the cultural needs of clients from various ethnic groups;
- Excellent interpersonal, communication and coping skills;
- Self-directed and disciplined to manage multiple caseloads and meet deadlines;
- Demonstrate flexibility and willingness to conduct emergency home visits as well as relief duties for off-site team members;
- Results oriented and capable of working independently as well as in fast-paced team environment;
- Must have basic computer skills (knowledge of Microsoft Outlook, word and excel);
- Possession of a valid Ontario driver’s license and access to a vehicle;

Work Conditions and Physical Capabilities:
· Monday to Friday from 8:30 to 4:30 with occasional evenings and weekends as required.
· General office environment.
· Significant amount of computer and telephone use.
· Can lift &/ or carry 6-13.5kg (15-30 lbs).
