Job Summary
Responsible and accountable to the Manager of Rehabilitation and Pharmacy Services to assist patients with personal care and activities of daily living in the assigned units by following the individualized care plans.
Key Accountabilities
- Performs all duties in accordance with the Mission Statement and policies and procedures of St. Joseph’s Health Centre as adopted from the Ministry of Health guidelines.
o Preserving the personhood of patients
o Encountering patients in a caring way
o Maximizing the individual’s participation and control
o Providing comfort and facilitating communication
o Encouraging and supporting individuals’ families to participate in care
- The Provision of Patient Care
o Assisting with personal hygiene (bathing, dressing, hair care, mouth care, nail care, and shaving)
o Assisting with nutrition (providing fluids and nourishments as required, meal set up, assistance with feeding, reporting of intake, and taking weights as required.)
o Assisting with urinary and bowel elimination (toileting, disposable briefs, emptying of urinary and colostomy drainage bags, documentation and reporting of output)
o Promoting comfort by assisting with positioning, mobility, and ambulation as per care plan (positioning and turning of patients ; using mechanical lifts appropriately; and reporting of any condition or mobility changes)
o Promoting skin integrity (providing good skin care through washing, positioning, and turning of patients; ensuring that environment is safe of obstacles to prevent injuries; completing basic wound care dressings; and reporting any changes in skin integrity). Immediately correcting if possible and reporting any situations that can or will effect the health and safety of the patients.
o Assisting with exercises
o Facilitating appropriate rest and socialization
o Accompanying patients who require an escort, to appointments
- Establishing and Maintaining Healthy Interpersonal Relationships
o Developing and preserving therapeutic PSW /SSW client relationships
o Developing and preserving collaborative relationships among the health care team and other programs/departments
- Ensuring the Quality of Health Care Practices
o Contributing to the quality of working life of peers and colleagues
o Improving the quality of life of patients through a patient and family centred care approach
o Learning and using new concepts and approaches in the care of adults
o Participating in improving the care provided to adults
o Consulting with peers and other health providers in approaches in the care of adults
o Contributing to and utilizing the established plan of care for patients
- Maintaining Professionalism and Work Role Competencies
o Attending and participating in educational in-services
o Demonstrating professionalism
o Providing services in a caring, respectful, and cost effective manner
o Participating in research and continuous quality improvement activities for improving the care provided to adults
o Recording documentation in an accurate and timely manner according to policies and procedures
Qualifications & Experience
- Personal support worker certificate or diploma from Ministry of Health of Ontario, Long Term Care Division approved educational facility
- Experience in long term care or hospital setting preferred
- Demonstrated written and verbal communication skills, and excellent interpersonal skills
- Current CPR certification preferred.
- Strong organizational skills and the ability to prioritize required.
- Ability to function as a team member in a collaborative environment required.
- Ability to accept direction and work independently required.
- Knowledge of general persuasive approach preferred.
