About the Opportunity
Hospice Huronia (HH) and the North Simcoe Ontario Health Team (NSOHT), in partnership with Georgian Bay General Hospital (GBGH), are seeking a Registered Nurse or Registered Practical Nurse (RN/RPN) for a unique 8-month contract as a Palliative Care Clinical Coach / Nurse Navigator.
North Simcoe is located on the shores of Georgian Bay and includes the communities of Christian Island, Midland, Penetanguishene, and Tay and Tiny Townships. Our local population of approximately 53,000 includes a diverse community with Francophone, First Nation, and Métis members.
Improving access to palliative care is a priority for the North Simcoe OHT. The successful candidate will work alongside a strong local healthcare community, including primary care, hospice, hospital, and Ontario Health atHome partners.
Job Summary
The Palliative Care Clinical Coach / Nurse Navigator plays a pivotal role in elevating the competency of front-line service providers at Georgian Bay General Hospital and across organizations in the North Simcoe community. This role supports the ongoing development of palliative care resources and infrastructure through knowledge exchange, coaching, and outreach.
Taking a consultative, educational, and mentorship-driven approach, the Clinical Coach / Navigator facilitates equitable access to palliative care services and helps individuals navigate complex healthcare systems. The role drives practice change with a focus on improving access to and quality of clinical care, aligned with the Quality Standard for Adults Living with a Life-Limiting Illness, the Ontario Palliative Care Competency Framework, and the Model of Care: Adults Receiving Palliative Care in Hospital Settings.
Key Responsibilities
Palliative Care Clinical Coach
- Build front-line provider competency in acute-palliative care, including early identification of palliative needs and a focus on Goals of Care, through case-based coaching and case reviews.
- Ensure organizations across long-term care, community, and hospital sectors have access to education, training, and resources aligned with the Adult Model of Care and Ontario Health directives.
- Directly engage front-line providers (PSWs, RNs, RPNs, physicians, and others) to foster collaboration and knowledge exchange.
- Deliver engaging seminars, in-services, and workshops to advance professional development in palliative care.
- Connect organizations and professionals to resources and supports for patients and caregivers, including the North Simcoe Palliative Support Program, Ontario Health @ Home, and the North Simcoe Muskoka Hospice Palliative Care Network.
- Guide palliative care quality improvement (QI) and practice change initiatives across acute care and community settings, including:
◦ Supporting QI initiatives related to the Adults in Hospital Settings Model of Care
◦ Building relationships that strengthen communication across interdisciplinary care teams
◦ Updating policies and procedures to sustain best practices
◦ Collaborating on the development of best practices in palliative care delivery
- Lead community engagement activities and develop materials that clearly communicate palliative care concepts and implementation plans.
- Foster regional collaboration and contribute to broader system integration.
- Build trusted relationships with key partners and participate in local, regional, and provincial committees and working groups as required.
- Keep the North Simcoe OHT and regional implementation team informed of community challenges and opportunities, and support implementation of the North Simcoe Palliative Care Pathway.
Palliative Nurse Navigation
- Provide coverage and support to the North Simcoe Palliative Nurse Navigator, ensuring enrolled patients receive consistent, timely symptom monitoring through the Remote Care Monitoring program.
- Identify and refer patients transitioning from hospital to community for primary palliative care monitoring, and educate families, patients, and providers on the program's benefits.
- Communicate assessment findings, care plans, and interventions clearly and effectively.
- Maintain complete, accurate, legible, and timely documentation of assessments, impressions, and care plans.
- Support patients and families in accessing advance care planning, psychosocial and spiritual care, and end-of-life planning.
- Collaborate with community partners, including OH@H Care Coordinators, Community Paramedicine, GBGH Dialysis Services, Chigamik Community Health Centre, North Simcoe Family Health Team, RVH Oncology, and Hospice Huronia, to provide wrap-around care and act as a liaison with external agencies.

- Work in compliance with the Occupational Health and Safety Act, reporting hazards and deficiencies in a timely manner.
What You Bring
Education & Experience
- RN or RPN in good standing with the College of Nurses of Ontario, with substantive palliative care experience (e.g., CAPCE).
- A minimum of three (3) years of experience in acute/palliative care nursing, with demonstrated experience in pain and symptom management and healthcare system navigation.
- Knowledge of the Quality Standard for Adults Living with a Life-Limiting Illness, the Excellent Care for All Act, the Compassionate Care Act, and other relevant legislation.
- Experience coaching and/or supervising clinical staff is preferred.
- Experience with large-scale quality improvement initiatives.
- Program development experience is considered an asset.
Knowledge & Skills
- Exceptional interpersonal, presentation, and communication skills, with proficiency in adult education principles and coaching methodologies.
- A commitment to equity-based, evidence-informed palliative care for all people in Ontario, regardless of age or illness.
- Advanced assessment, critical thinking, problem-solving, and crisis management abilities.
- Proficiency with computer systems, including PS Suite EMR and Accuro EMR, and willingness to learn the CloudDx interface for Remote Care Monitoring.
- Ability to work well under pressure and exercise sound judgment in difficult situations.
- Ability to work independently and as part of a team.
- Strong clinical coaching and debriefing skills with individuals and teams.
- Strong consultation skills, with expert knowledge of consultation models and their application across individuals, teams, and community service providers.
- Commitment to engaging with First Nations, Indigenous, Métis, and urban Indigenous people as they self-determine their approach to palliative care.
- Knowledge of community resources in the North Simcoe region is considered an asset.
Other Requirements
- Vulnerable Sector Search completed within the last six (6) months.
- RNAO membership or valid liability insurance.
- Ability to use Personal Protective Equipment (PPE) as needed.
