About the position:
We are seeking a compassionate, reliable, and motivated Community Resource Worker II (CRW II) to support three adult male individuals with developmental disabilities, mental health disorders, and complex behaviors in their home and throughout the community.
In this role, you will provide person-centered support that promotes independence, dignity, and quality of life while taking a lead role in case management and service coordination. Responsibilities include assisting with daily living activities, implementing individualized support strategies, supporting positive behavior outcomes, encouraging community inclusion, and helping individuals achieve their personal goals.
As a CRW II, you will be responsible for developing, monitoring, and evaluating individual support plans and goals, maintaining accurate documentation, completing progress reports, and ensuring services are delivered in accordance with organizational standards and best practices. You will work closely with individuals served, families, guardians, community partners, and interdisciplinary professionals to coordinate support, advocate for individual needs, and ensure meaningful outcomes are achieved.
The successful candidate will demonstrate strong leadership, communication, organizational, and problem-solving skills. The ability to assess individual needs, develop person-centered goals, monitor progress, and provide guidance and mentorship to team members will be essential in this role. Experience supporting individuals with developmental disabilities, mental health concerns, complex behavioral needs, and community-based living is required.

Hours: Monday, Tuesday and Wednesday 3pm-9 pm, Thursday & Friday 3 pm- 10 pm, total of 34 hours per week
Location: Edmonton, Alberta.
What you’ll do:
· Connect with and develop helping relationships with individuals with disabilities.
· Acting as a strong advocate for individuals in the community.
· Work effectively as a team with the individual and their support team to help achieve their goals.
· Completion of daily documentation.
· Case management; including support plan development and implementation
· Support and participate in recreation and leisure activities.
· Perform administrative and cleaning duties relating to the operation of the home.
· Participate in on-going program development.
What you bring:
· Completion of a High School Diploma.
· Please note that applicants with a degree or diploma in Social Work from a university accredited by the Alberta College of Social Workers are required to be registered with the College (RSW) before starting or obtain registration within three months of hire.
· One (1) year experience working within the Human Services field.
· Equivalencies may be considered.
· Security Clearance Check; including the Vulnerable Persons Sector Check (from within the last 6 months).
· Emergency First Aid-Level A CPR + AED.
· Valid Class 5 driver’s license, current driver’s abstract, reliable vehicle, $2 Million auto insurance and comfort with driving to various locations are required. Three years’ driving experience with a valid class 5 license is an asset.
· Strong verbal and written communication skills.
· Computer/Word processing skills.
· Ability to implement approved restrictive practices.
· Ability to assist with personal care
