Trevor Frances Recruitment is recruiting on behalf of a respected long-term care organization seeking a Nurse Practitioner (NP) to join their clinical leadership team.
In this role, you'll practice to the full scope of your licence while providing high-quality, resident-centred care across multiple long-term care homes. You'll collaborate with physicians, nursing leaders, and interdisciplinary healthcare professionals to improve resident outcomes, support clinical excellence, and contribute to ongoing quality improvement initiatives.

What You'll Do
As the Nurse Practitioner, you'll provide advanced clinical care while supporting evidence-based practice and continuous improvement across the organization.
Clinical Care
- Complete comprehensive health assessments and develop diagnoses.
- Develop, implement, and evaluate individualized care plans.
- Order, interpret, and follow up on diagnostic tests.
- Prescribe, adjust, and monitor medications and other therapeutic interventions within your scope of practice.
- Collaborate with physicians and interdisciplinary team members to coordinate resident care.
- Arrange referrals to specialists and community resources as needed.
- Support hospital admissions and discharges to ensure continuity of care.
- Provide palliative and end-of-life care.
- Promote resident safety, quality of life, and person-centred care.
- Support residents and families through education and clinical guidance.
Clinical Leadership
- Participate as a member of the Centre Management Team.
- Support quality improvement initiatives and implementation of clinical best practices.
- Contribute to the development and review of policies, procedures, and medical directives.
- Provide clinical consultation for complex resident care situations.
- Promote evidence-informed practice throughout the organization.
Education & Mentorship
- Mentor and support Registered Nurses, Registered Practical Nurses, Personal Support Workers, and nursing students.
- Identify learning opportunities and provide clinical education.
- Encourage the use of current research and best practices in resident care.
- Develop educational resources for staff, residents, and families.
Quality Improvement & Research
- Participate in clinical research and quality improvement initiatives.
- Evaluate resident outcomes and recommend improvements.
- Support organizational projects focused on nursing excellence and resident care.
Qualifications Required
- Current registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (Extended Class).
- Master's Degree in Nursing (MScN).
- Strong clinical assessment and diagnostic skills.
- Excellent communication and collaboration skills.
- Strong critical thinking and problem-solving abilities.
- Effective organizational and time management skills.
- Experience using electronic health records (PointClickCare or similar systems).
- Knowledge of current long-term care legislation, standards, and best practices.
- Ability to maintain confidentiality and exercise sound professional judgment.
- Successful Police Vulnerable Sector Check.
Assets
The following qualifications are considered assets but are not required:
- Experience in long-term care or geriatric care.
- Gerontological Nursing Certification.
- Palliative Care or End-of-Life Care education.
- GPA, PIECES, or other seniors-focused education.
- Ability to communicate in more than one language.

Pay: $125,000.00-$145,000.00 per year
