Reporting to the Program Manager, PPDD, Team Coordinators provide direct care and support to the individuals that receive funding through Disability Services/PDD in community settings within the Shared Living and Supportive Independent Living models. The supports provided promote well-being, independence, and community connectedness while assisting individuals to achieve goals and meet their personal outcomes. The Team Coordinator is key in the success of the home working closely with the Program Manager to ensure that both individuals and employees reach their full potential.
Team Coordinators are required to provide physical, emotional, and/or behavioral support to persons with developmental disabilities in all areas of daily living, recreation/leisure activities and community inclusion. Client support includes personal care, lifting and transferring, behavioral intervention, assistance with meal preparation, and housekeeping activities.
Educational and other Qualifications
- Rehabilitation Practitioner Diploma or an equivalent combination of education and experience
- Valid First Aid and CPR Certification
- Food Safe certification

Work Experience
- Two years front line experience working with individuals with developmental disabilities
- Supervisory experience is preferred
- Ensures the site is safe by coordinating GSS safety administration practices such as: fire drills, site maintenance, and completing documentation
- Recognizes, develops, and leads behavioral management processes and documentation
- Provides direct care and support based on individuals needs
- Responsible for the day to day operations, such as: cleaning, grocery shopping, menu planning, ordering materials, and supplies and ensuring effective stewardship of resources
- Responsible for ensuring adherence to the PPDD Financial Guidelines
- Ensures that all documentation processes are completed and submitted according to requirements
- Fosters and maintains open lines of communication with individuals, families, guardians, GSS, other service providers, and community professionals
- Accountable for providing performance feedback/coaching to team members, including performance appraisals to Community Service Workers and casual employees
- Promotes the well-being of staff members in regards to health and OH&S and ensures access to the necessary resources
- Coordinates scheduling and the completion of timesheets
- Prepares for and facilitates individual annual service planning meetings
- Conducts team meetings, participates in committees as assigned, attends coordinator meetings, and other off site meetings as required
- Identifies, develops, and implements protocols related to care in consultation with PPDD RN and Program Manager
- Supports and provides feedback to employees through orientation and probationary period
- Coordinates training requirements for employees within home in consultation with Learning and Development
- Participates in the process of individuals transitioning into PPDD services in consultation with Program Manager, PPDD RN, and external stakeholders
- Maintains a high standard of cleanliness, keeping the work environment consistently tidy
- Ensures that all property maintenance concerns/hazards are reported and followed up in a timely manner including all documentation processes
- Adheres to all infection control practices and procedures
- Ensures an environment and resources which nurture spirituality
- Other related duties as assigned
Requirements:
- A current and clear Criminal Records Check Form and if applicable a Vulnerable Sector Check that has been obtained no more than 90 calendar days prior to the hire date.
