The Role
The Care Coordinator provides leadership and oversight to ensure high‑quality, person‑centred care that supports physical, emotional, psychological, and spiritual well‑being. This role leads care coordination activities, serves as the primary MDS/RAI Coordinator for the Home, and works closely with interdisciplinary teams to support excellent clinical outcomes.
The Care Coordinator acts as a key liaison with the people who live with us, their families and healthcare partners and represents the organization within the broader community.
This is an excluded (management) position offering the opportunity to lead with the purpose and make a meaningful impact in seniors’ care. The hours of work are Monday to Friday, 08:30 – 16:30. Some evening or weekend work may be required.
What You’ll Do
This role provides clinical and operational leadership to ensure safe, high-quality, person-centered care.
As the Care Coordinator, you will be responsible for:
· Leading and coordinating resident care using the nursing process, ensuring compliance with legislation, licensing requirement, and organizational policies
· Supervising and supporting unionized nursing and care staff, including daily staffing, scheduling, orientation, education, and performance management, in collaboration with Human Resources and within collective agreements.
· Coordinating admissions, discharges, transfers, care planning, physician orders and medication management assessments in collaboration with the interdisciplinary team, physicians, pharmacy, and families.
· Supporting quality, safety, and risk management activities, including infection control, wound care, medication safety, incident reporting, and emergency response as required.
· Promoting a safe, respectful, and home‑like environment that upholds the Resident Bill of Rights and addresses resident and family concerns

What You Bring
Education & Experience
· Current LPN registration – in good standing with the BCCNM
· Experience in residential or long-term care, geriatrics, or a complex, multidisciplinary healthcare setting
· Leadership or team-lead experience preferred
· Working knowledge of interRAI, long-term care legislation, regulations, and collective agreements (BCNU and HEU)
Skills & Competencies
· Strong leadership, communication, and interpersonal skills, with the ability to teach, coach, and support staff.
· Demonstrated ability to plan, organize, prioritize, and manage multiple responsibilities in a fast‑paced environment.
· Strong clinical knowledge of long‑term care, including palliative care, and commitment to continuous learning and quality improvement.
· Ability to work effectively both independently and as part of an interdisciplinary team, and to engage residents and families with empathy and respect.
· Proficient computer skills, including Microsoft Word, Excel, PowerPoint, and Outlook.
Additional Requirements
· Eligible to work with vulnerable adults under the BC Criminal Records Review Program
· Proof of negative TB Screen and immunizations status.

Pay: $88,268.00-$99,302.00 per year
